Consultations by Appointment Only - Hours May Vary

SoNaturelle by Milan does not use "electronic billing". All payments must be made with cash or valid credit card and/or bank debit card. After scheduling your wellness consultation you will receive an email or phone call for confirmation. In order to hold your begin date we require a signed contract as well as a 30% deposit which is added towards your 1st Consultation.

Please try and schedule your consultation at least 48 hours in advance. For Bridal Wellness, we recommend booking at least 3 months in advance to allow a maximum amount of time for healthy weight loss prior to your ceremony and honeymoon.

In order for us to partner together, it is important to start and end our session on time. Routine “sign & return” forms will be provided at the start of your visit. To avoid a scheduling conflict or “missed appointment” clients should arrive at least 15-20 minutes prior to their scheduled appointment. If you are delayed for your appointment, your session may be condensed and charged for the full service depending on the minutes that you reserved, and the schedule of the day. A 24-hour advance notice is required for appointment cancellations. A 2nd “re-schedule request” will count as one missed appointment.**

** Please notify us at least 24 hours in advance if it becomes necessary to reschedule your appointment. There is a cancellation fee of $25.00 for missed appointments without proper notification (no shows); or multiple rescheduled cancellations. Products and services are non-refundable.

Privacy

I have chosen to remain a “non-covered entity” under the Health Insurance Portability and Accountability Act of 1996. Actively participating in HIPAA would mean that hundreds of government agencies would have virtually unlimited access to your private records without your consent. I will not share your information with a third party. Your records will be released ONLY with your consent. “SoNaturelle by Milan” does not file electronic insurance claims.
